Today's phrase is the same age as me, it was first recorded in 1974. And there is no origin that can be found but it seems that it was probably birthed in classrooms when teachers were making sure all the students were on the same page in their textbooks for the lesson.
We use it now, outside of the classroom, to mean similar thinking or understanding.
